Humeral bone grafting in stemless shoulder arthroplasty

چکیده
Stemless shoulder arthroplasty with metaphyseal fixation of the humeral component is increasingly used in the treatment of primary or secondary osteoarthritis, achieving significant pain relief and improving both range of motion and patient satisfaction [1, 4, 6]. The majoradvantagesof the stemlesshumeral design are preservation of humeral bone stock, anatomical reconstruction regardless of humeral malalignment, fewer stem-related complications, and ease of revision [2]. Hawi et al. recently showed good long-term clinical and radiological outcomes with a revision rate of approximately 7% without humeral implantrelated complications [6]. Currently, poor bone quality, including osteoporosis or metaphyseal cystic changes, is described to be a contraindication for stemless shoulder prostheses [5]. Thus, it is generally recommended to switch to a stemmed prosthesis to provide bone ingrowth and primary stability. The purpose of the following technique is to embrace the advantages of the stemless design when treating severe primary or secondary osteoarthritis, even in the presence of a large humeral bone defect, using the Eclipse prosthesis (Arthrex, Naples, FL, USA) combined with a humeral autograft (. Bone Graft Substitute Provides Metaphyseal Fixation for a Stemless Humeral Implant.
Stemless humeral fixation has become an alternative to traditional total shoulder arthroplasty, but metaphyseal fixation may be compromised by the quality of the trabecular bone that diminishes with age and disease, and augmentation of the fixation may be desirable. The authors hypothesized that a bone graft substitute (BGS) could achieve initial fixation comparable to polymethylmethacrylate (P...
متن کاملIs the stemless humeral head replacement clinically and radiographically a secure equivalent to standard stem humeral head replacement in the long-term follow-up? A prospective randomized trial.
BACKGROUND Stemless humeral head replacement represents a young generation of shoulder arthroplasty. This study evaluated the differences of this new stemless design compared with the fourth-generation standard stemmed design. METHODS Total shoulder arthroplasty was performed in 20 patients with a stemless shoulder prosthesis (group 1) and in 20 patients with a standard stem humeral head repl...
متن کاملAnatomic stemless shoulder arthroplasty and related outcomes: a systematic review
BACKGROUND The latest generation of shoulder arthroplasty includes canal-sparing respectively stemless designs that have been developed to allow restoration of the glenohumeral center of rotation independently from the shaft, and to avoid stem-related complications. The stemless prosthesis design has also recently been introduced for use in reverse arthroplasty systems. METHODS We systematica...
متن کاملRadiological changes do not influence clinical mid-term outcome in stemless humeral head replacements with hollow screw fixation: a prospective radiological and clinical evaluation
BACKGROUND Stemless shoulder arthroplasty is a fairly new concept. Clinical and radiological follow-up is essential to prove implant safety and concept. This prospective single-centre study was performed to evaluate the influence of radiological changes on clinical mid-term outcome following stemless humeral head replacement with hollow screw fixation. METHODS Short- and mid-term radiological...
متن کاملStability of Reverse Shoulder Humeral Implants in a Proximal Humerus Bone Loss Model
The treatment of proximal humeral bone loss with its associated pain and loss of function continues to be a challenge for the orthopaedic surgeon. Current options for this condition include hemiarthroplasty or total shoulder arthroplasty with the addition of bone grafting. Unfortunately, due to residual instability, these options have not given the desired results. Reverse shoulder arthroplasty...
